"It gives me great pleasure to write this introduction to our second monograph on the subject of Coronary Artery Disease. Just ten years ago, I conceived the idea of starting a society (SASAT) to create awareness, develop educational and preventive programs. I am quite impressed with what we have accomplished in this short period. We have made significant progress in creating awareness and the development of educational programs. SASAT has organized five successful international symposiums on Atherosclerosis and Thrombosis in India, since 1993. When we started this work ten years ago, we were concerned with the disease of the heart. Soon we realized, that it is more a disease of the arteries that supply precious blood to the heart, than the disease of the heart. Now it is well accepted, that the health of the endothelium and the blood vessels affects the overall vascular function. We now know, that the vascular disease affects not only the heart and the brain, but also miles of blood vessels throughout the
human body. To a great extent, vascular function or dysfunction is modulated by the chemistry and pathophysiology of circulating blood cells and the endothelial cells that line the vessel wall.
This awareness has not only created a new discipline "vascular medicine", but has brought together multiple disciplines, to work together, to conduct research and take care of patients with vascular disease. Hence, we have emphasized in this book several times, the importance of endothelial function and the ill effects of platelet and endothelial dysfunction. Altered function of these cells leading to diminished production of vasodilators and increased generation of vasoconstrictory metabolites, precedes the manifestation of the vascular diseases. Therefore, the health of endothelium and the bioactive state of the circulating blood cells and the platelets is as important or more important, than the presence of common risk factors such as cholesterol and triglycerides. There is a considerable evidence to suggest, that early diagnosis of vessel wall abnormality and appropriate intervention, will make a big difference in the management of these chronic diseases. Hence, the emphasis of SASAT is on prevention of these diseases by early diagnosis of the disease and better management of the risk promoters."Gundu FIR Rao
