This meeting provides an opportunity to discuss the important issue in epilepsy prescribing practice of switching from branded to generic antiepileptic drugs (AEDs). For patients who receive a generic prescription, switching between generic products commonly occurs when their prescriptions are dispensed, and the impact of this should also be considered. The discussion should be as much about considering the role of physicians and pharmacists as about the needs of patients, who often do not have much choice in the drugs that they receive.
'A recent paper' suggests that ~he practice of substituting branded with generic AEDs is well established. The US Food and Drug Administration (FDA) claims that it can give assurances that any approved generic drug should match the branded drug it replaces in terms of clinical results and safety profile.' This claim is made despite the often voiced concerns by physicians and pharmacists that there is potential for non equivalence between branded and generic drugs, which in some cases may be due to the narrow therapeutic range, low water solubility, and non linear pharmacokinetics of AEDs.
