In the last five years, the themes of conferences on cystic fibrosis have focused on two main areas: early events and established pseudomonal infection.

Issues with respect to early events include:
-How is Pseudomonas acquired?
-Which Pseudomonas species are a problem?
-Does cross infection occur?
-How much Pseudomonas comes from the environment and how much from other patients in hospital?
-Can Pseudomonas be eradicated early? If so, how can we achieve this?

Once pseudomonal infection is established, other issues need to be considered, and the current main themes are:
-biofilms
- hypermutaters
-intensity of antibiotic treatment, particularly with the problem of multi resistant organisms.

Within each of these themes, a number of different antibiotics are seeking a place and a number of different treatment strategies are being developed, but none has a good evidence base. At medical school, training about colistin only reveals that this drug is bad for the central nervous system and the renal system. Much of the information available about colistin, in fact, has been uncovered largely by clinical experience.
The aim of this meeting is to provide up to date information on the mechanism of action of colistin and its role in the themes described, to understand better its role in the treatment of cystic fibrosis and to discuss how doctors manage infections in patients with cystic fibrosis in clinical practice. In situations with a lack of published evidence, shared experience is the key to improving the management of patients and that is the focus of this meeting.
